AI Agents Are Already Failing in Production — and Most Companies Are Still Deploying Them Anyway

A survey of 157 enterprises found that half had already shipped an AI agent that passed internal tests and then failed a real customer in production. Only one in twenty companies fully trusts their automated evaluation systems today. Despite this, two-thirds are already deploying or actively moving toward deploying agent changes directly to production.
